Navigating the “Now What?” After Physical Therapy

Humana already had a program to help injured employees get back on their feet. But they noticed a gap: Even after physical therapy, many were still struggling to fully thrive.

Too often, employees were cleared to return, but didn’t feel physically or mentally ready to work at full capacity. And with vendors working in silos, the support system often fell short.

“When vendors don’t work well together… you lose the trust. You lose that community environment. And then you start from square one,” said Christa T., Humana’s Workplace Strategies & Services Lead.

What Humana employees needed wasn’t just another program. They needed a true partner who could connect the dots, work hand-in-hand with their teams and vendors, and help employees finally feel and perform like themselves again.

From Repair to Ready: A Cohesive Path to Full Strength

Seamless Clinical-to-Coaching Handoff

Exos and Humana’s physical therapy team co-designed a Return to Activity program to fill the post-PT gap. The goal: to help employees sustainably return to 100% capacity, or even better than their pre-injury selves.

This ensured that after formal rehabilitation, associates didn’t face recovery alone. Instead, they transitioned seamlessly to Exos performance coaches. This removed the traditional friction between medical discharge and full return to function.

Every participant began with a full-body assessment, after which the coach designed a personalized progression plan aligned to their goals.

Goal-Aligned, Coach-Led Progression

Exos coaches aimed higher than recovery to the norm. By setting stretch goals and calibrating programming based on daily feedback, they built trust and elevated outcomes.

Meanwhile, Gameplans were flexible to each employee’s needs. Coaches knew when to push and when to pull back with each participant, empowering them to surpass self-imposed limits.

 

A Collaborative Ecosystem of Care

Crucial to the program’s success was vendor alignment. As one Humana lead noted, “You want a seamless handoff, but you also want transparency.”

So, Exos integrated with Humana’s broader well-being infrastructure and collaborated across clinical and community teams to build a trusted care continuum. All parties involved were clear on the plans, and bought in on the execution.

This partnership ensured every participant received world-class care from beginning to end.
